Light chain myeloma (LCM) in India presents differently than in Western studies, with distinct clinical features. Indian LCM patients show comparable survival rates to other multiple myeloma subtypes, suggesting effective treatment protocols.
Area of Science:
- Hematology
- Oncology
- Clinical Research
Background:
- Light chain myeloma (LCM) accounts for 15-20% of multiple myeloma (MM) cases globally.
- Western studies link LCM to anemia, renal failure, and poorer survival, possibly due to poor differentiation.
- Understanding LCM characteristics in diverse populations is crucial for tailored treatment strategies.
Purpose of the Study:
- To investigate the clinical and hematological profiles of Indian LCM patients.
- To compare the prognostic outcomes of Indian LCM patients with IgG and IgA myeloma subtypes.
- To identify unique characteristics and survival patterns of LCM in the Indian subcontinent.
Main Methods:
- Retrospective observational study of 104 newly diagnosed multiple myeloma patients.
- Classification based on International Myeloma Working Group criteria (2016) and staging via ISS.
- Comparison of clinical data between IgG, IgA, and light chain myeloma subgroups.
Main Results:
- LCM patients showed significant correlation with hypercalcemia and elevated serum-free light chain ratios.
- IgA myeloma patients were associated with anemia and lower serum albumin levels.
- No significant differences observed in lactate dehydrogenase, renal failure, bone lesions, ISS score, chemosensitivity, or 1-year progression-free survival among subtypes.
Conclusions:
- Indian LCM patients exhibit a distinct clinico-hematological profile compared to global cohorts.
- Prognostic outcomes for Indian LCM patients are not inferior to other myeloma subtypes.
- Standardized treatment regimens may contribute to comparable survival rates in Indian LCM patients.
