Summary

Background:

  • Masson's tumor, or intravascular papillary endothelial hyperplasia (IPEH), is a benign intravascular neoplasm.
  • Histologically, it features papillary endothelial projections within a vessel lumen, associated with thrombi.
  • CD 31 and CD 34 immunohistochemistry aid in identifying vascular origin.

Observation:

  • A 54-year-old male presented with a 3-week history of a right lateral neck mass.
  • CT revealed a 20mm×9mm cystic mass with a draining vessel.
  • Surgical excision and subsequent pathology confirmed a thrombosed vein with papillary structure.

Findings:

  • The resected mass showed features consistent with Masson's tumor (IPEH).
  • No evidence of atypia, necrosis, or perivascular invasion was noted.
  • The patient experienced an uncomplicated recovery with no recurrence at 8 months.

Implications:

  • IPEH can be distinguished from angiosarcoma by the absence of atypia, necrosis, and perivascular extension.
  • Surgical excision is the recommended treatment for Masson's tumor.
  • This case highlights an unusual presentation of Masson's tumor in the neck.
