Summary

Primary pulmonary synovial sarcoma, a rare lung cancer, can present as a calcified nodule on CT scans. This finding challenges the assumption that calcification in lung nodules always indicates a lower cancer risk.

Background:

  • Primary pulmonary synovial sarcoma is a rare malignancy, with the lung being the most commonly affected organ.
  • Intratumoral calcification is an uncommon feature in pulmonary synovial sarcomas compared to soft tissue sarcomas.
  • Calcification within a lung nodule is often associated with a reduced risk of lung cancer.

Observation:

  • A case report details a 52-year-old male with an asymptomatic solitary pulmonary nodule.
  • Computed tomography (CT) revealed a well-defined nodule with punctate and eccentric calcification in the right lower lobe.
  • Enhanced CT and 18F-fluorodeoxyglucose positron-emission tomography indicated a malignant tumor.

Findings:

  • Histopathological examination confirmed monophasic spindle-cell synovial sarcoma with hyalinized collagen bands and calcifications.
  • Genetic analysis identified the SYT-SSX2 fusion gene, consistent with synovial sarcoma.
  • The tumor presented as a calcified nodule, an atypical feature for this condition in the lung.

Implications:

  • Primary pulmonary synovial sarcoma should be considered in the differential diagnosis of solitary pulmonary nodules with calcification.
  • This case highlights the importance of comprehensive evaluation for lung nodules, even those with calcification.
  • Radiological and pathological findings underscore the variability in presentation of rare thoracic malignancies.
