Summary

Atropine penalization and conventional occlusion are equally effective for treating amblyopia (impaired vision). Atropine offers better adherence and quality of life, despite some adverse effects, and is more cost-effective for amblyopia treatment.

Background:

  • Amblyopia, or 'lazy eye,' is reduced vision in one or both eyes without a clear physical abnormality.
  • Current treatments include conventional occlusion (patching) and atropine penalization.

Purpose of the Study:

  • To systematically review and compare the effectiveness and safety of conventional occlusion versus atropine penalization for amblyopia treatment.
  • To synthesize evidence from randomized and quasi-randomized controlled trials.

Main Methods:

  • Systematic search of multiple databases including CENTRAL, MEDLINE, Embase, LILACS, ClinicalTrials.gov, ISRCTN, and WHO ICTRP.
  • Inclusion of seven trials (five RCTs, two quasi-RCTs) with 1177 amblyopic eyes.
  • Independent screening, data abstraction, and risk of bias assessment by two reviewers.

Main Results:

  • Atropine penalization demonstrated comparable effectiveness to conventional occlusion in improving visual acuity in amblyopic eyes.
  • Similar visual acuity improvements were observed across various time points (5 weeks to 10 years).
  • Atropine showed better adherence and quality of life, but higher rates of mild sound eye vision reduction and light sensitivity; patching caused more local irritation. Both were well-tolerated.

Conclusions:

  • Both conventional occlusion and atropine penalization effectively improve visual acuity in amblyopia.
  • Atropine penalization is a viable alternative to occlusion, offering comparable efficacy with potential benefits in adherence and quality of life.
  • Further research may be needed to fully elucidate long-term outcomes and optimal treatment strategies.
