Background:

VACTERL association is a rare, sporadic condition involving multiple congenital malformations.
Situs inversus totalis (SIT) and esophageal malformations are infrequently associated with VACTERL.
This report documents the first North American case of VACTERL association with SIT.

Purpose of the Study:

To report the first North American case of VACTERL association with Situs Inversus Totalis (SIT).
To emphasize the diagnostic challenges and clinical considerations for respiratory distress in infants with complex congenital anomalies.

Main Methods:

Case presentation of an infant diagnosed with VACTERL association and SIT.
Review of physical examination findings and imaging studies.

Main Results:

The infant presented with a rare combination of VACTERL malformations and SIT.
