Background:

  • Epithelioid sarcoma (ES) is a rare and aggressive malignancy.
  • Limited data exists due to its rarity, necessitating comprehensive reviews.

Purpose of the Study:

  • To review the traits, prognosis, and survival features of epithelioid sarcoma.
  • To analyze a large cohort using a national cancer database and literature.

Main Methods:

  • Data from the Survival, Epidemiology, and End Results (SEER) database and literature were analyzed.
  • 1, 5, and 10-year Disease Specific Survival (DSS) rates and hazard ratios (HR) were calculated.
  • Cases were stratified into pre-2000 and post-2000 diagnostic groups.

Main Results:

  • A total of 1990 ES cases were analyzed (998 from SEER, 992 from literature).
  • Age, anatomical site, grade, TNM stage, treatment, and diagnosis year predicted survival.
  • Overall 5- and 10-year survival rates were 60.4% and 50.2%, respectively, with recurrence at 63.4% and metastasis at 40.3%.

Conclusions:

  • This study represents the largest cohort of epithelioid sarcoma to date.
  • Factors influencing better outcomes exist, despite the generally poor prognosis.
  • A concerning trend of worsening survival post-2000 requires further investigation.
