Background:

  • Anterior cruciate ligament (ACL) injuries are common, impacting knee function.
  • Reconstruction surgery (ACLr) aims to restore function, but outcomes vary.
  • Quantifying functional recovery requires understanding the relationship between objective and subjective measures.

Purpose of the Study:

  • To quantify the relationship between objective and subjective functional measures.
  • To determine if pre-operative (ACLd) measures predict post-operative (ACLr) outcomes.
  • To assess the predictive capability of deficient-state measures for reconstructed-state function.

Main Methods:

  • Twenty participants with ACL injuries underwent functional tests (hop, side cut) pre- and post-reconstruction (10 months).
  • Subjective scores (Tegner, Lysholm, IKDC, KOOS, KNEEs) were correlated with objective measures (peak knee flexion, extensor moment, stiffness, KJCE).
  • Regression analyses identified predictive relationships between ACLd and ACLr scores.

Main Results:

  • Relationships between objective and subjective measures were task and ACL status dependent.
  • Knee joint center excursion (KJCE) and stiffness most frequently correlated with subjective scores.
  • Peak knee flexion angle best predicted ACLr state from ACLd state (adj. R² = 0.4-0.66).
  • KOOS-ADL showed strongest correlations (r = 0.70-0.77), while Tegner had greatest predictive power (OR: 1.46-1.86).

Conclusions:

  • Objective and subjective measures of knee function show variable, sometimes strong, correlations.
  • Pre-operative objective measures are more strongly correlated with and better predictors of post-operative outcomes than subjective measures.
  • This highlights the importance of objective biomechanical assessments in predicting ACL reconstruction success.
