Summary

Iodine-stained fragmented thromboembolism (ISFT) is a rare finding after stroke treatment. Recognizing ISFT helps detect residual clots and avoid misinterpreting them as calcifications.

Background:

  • Iodine-stained fragmented thromboembolism (ISFT) is a rare phenomenon observed after mechanical thrombectomy or carotid stenting.
  • Its imaging appearance and pathophysiology require further elucidation.

Purpose of the Study:

  • To describe the imaging characteristics of ISFT.
  • To discuss the underlying pathophysiology of ISFT.

Main Methods:

  • Retrospective review of 49 patients undergoing mechanical thrombectomy for acute stroke over one year.
  • ISFT defined as arterial luminal filling defect with Hounsfield density ≥ iodine on CT.
  • Follow-up CTA/MRA assessed recanalization, infarction, and ISFT fate.

Main Results:

  • ISFT identified in 8 of 49 patients (16.3%) across various cerebral artery locations (M1-M3, A1-A2).
  • Vessel recanalization occurred in 5 patients with ISFT on follow-up.
  • Persistent ISFT and vessel occlusion noted in 2 patients.

Conclusions:

  • ISFT likely results from mechanical disruption of thromboemboli, with porous fragments retaining contrast.
  • Identifying ISFT is crucial for detecting residual thromboembolism.
  • Avoid misinterpreting ISFT as calcified thromboembolism.
