Summary

Pyoderma gangrenosum (PG) is often linked to hematologic malignancies, particularly myelodysplastic syndrome (MDS). Patients with PG require evaluation for these cancers, as MDS is the most frequent association.

Background:

  • Pyoderma gangrenosum (PG) is a severe neutrophilic dermatosis.
  • Its association with systemic diseases, especially hematologic malignancies, is not fully understood.

Purpose of the Study:

  • To systematically review the characteristics and temporal relationship of pyoderma gangrenosum with hematologic malignancies.
  • To understand the prevalence and types of hematologic cancers associated with PG.

Main Methods:

  • Systematic review of 340 cases from 279 publications.
  • Searched MEDLINE, EMBASE, Scopus, and Web of Science up to December 12, 2018.
  • Data extraction by two independent reviewers.

Main Results:

  • Myelodysplastic syndrome (MDS) was the most common associated hematologic malignancy.
  • Ulcerative PG subtype and multifocal distribution were predominant.
  • MDS cases typically preceded PG, while monoclonal gammopathy of undetermined significance (MGUS) cases followed PG.

Conclusions:

  • Patients with PG should be screened for underlying hematologic malignancies.
  • MDS is the most frequently observed hematologic malignancy linked to PG.
  • Limitations include reporting bias and evolving classification of hematologic malignancies.
