    Background:

    • Meningococcemia, a serious bacterial infection, can lead to severe complications.
    • Endophthalmitis is an uncommon but severe intraocular inflammation.
    • Antimicrobial therapies have significantly reduced the incidence of endophthalmitis secondary to meningococcal disease.

    Observation:

    • A pediatric case presented with endophthalmitis as the primary symptom.
    • Clinical signs included bilateral ocular inflammation, a fixed and dilated right pupil, obscured fundus view, and hypopyon in the right anterior chamber.
    • The patient showed gradual improvement following antibiotic treatment.

    Findings:

    • This case demonstrates that meningococcemia can manifest with unusual sites of infection, specifically endophthalmitis.
    • The presentation of endophthalmitis as the initial complaint in meningococcemia is rare.
    • Prompt antibiotic therapy led to clinical improvement.

    Implications:

    • Reinforces the need to consider meningococcemia in the differential diagnosis of pediatric endophthalmitis.
    • Suggests that meningococcemia, despite its rarity, can present with atypical localizations.
    • Highlights the importance of early diagnosis and treatment of meningococcal infections to prevent severe ocular complications.
