This study shows that rumination-focused cognitive behavioural therapy (RfCBT) significantly reduces depressive symptoms, rumination, and worry in patients with chronic and treatment-resistant depression. These positive outcomes were maintained at follow-up, suggesting RfCBT is a promising treatment approach.
Area of Science:
- Psychiatry
- Clinical Psychology
- Cognitive Behavioral Therapy
Background:
- Treatment-resistant depression affects a significant portion of patients.
- Chronic depression impacts approximately 20% of those treated.
- Rumination is a key mechanism in depression development and maintenance.
Purpose of the Study:
- To assess the feasibility of individual rumination-focused cognitive behavioural therapy (RfCBT).
- To evaluate RfCBT for patients with chronic and treatment-resistant depression.
Main Methods:
- 10 patients with chronic/treatment-resistant depression received 12-16 individual RfCBT sessions.
- Primary outcome: depressive symptoms (Hamilton Depression Scale).
- Secondary outcomes: self-reported rumination and worry.
Main Results:
- Significant reductions in depressive symptoms (p < 0.05), rumination (p < 0.01), and worry (p < 0.5) were observed.
- 50% of participants showed reliable change in depressive symptoms post-treatment.
- Symptom improvements were sustained at 3-month follow-up.
Conclusions:
- RfCBT demonstrated significant reductions in depressive symptoms for a small sample of difficult-to-treat patients.
- The findings are promising for chronic and treatment-resistant depression.
- Further systematic evaluation of RfCBT is warranted.
