Background:

  • Helical tomotherapy efficacy for chemo-refractory liver metastases requires evaluation.

Purpose of the Study:

  • To assess the efficacy of helical tomotherapy in patients with chemo-refractory multiple liver metastases.
  • To evaluate the adverse events associated with helical tomotherapy in this patient group.

Main Methods:

  • A prospective, single-institutional study included 45 patients with 3-10 liver metastases.
  • Patients received a prescribed dose of 55 Gy in 25 fractions.
  • Toxicity and survival outcomes were analyzed, with a median follow-up of 8 months.

Main Results:

  • 93% of patients completed treatment; median survival was 8 months with a 1-year survival rate of 29%.
  • Local control was achieved in 33% of tumors at 6 months; 31% showed tumor marker reduction.
  • Grade 3 lymphocytopenia and fatigue were the most common toxicities; no radiation-induced liver disease (RILD) was observed.

Conclusions:

  • Helical tomotherapy is a feasible and potentially effective treatment for chemo-refractory multiple liver metastases.
  • Consideration of tomotherapy in combination with first-line systemic chemotherapy is recommended.
  • Prognostic factors for poorer outcomes include primary pancreatic cancer, extrahepatic metastases, and low NLR/MLR.
