Cladribine effectively treated refractory myasthenia gravis (MG). Most patients showed significant improvement, reducing steroid needs and avoiding other intensive therapies without adverse events.
Area of Science:
- Neurology
- Immunology
- Pharmacology
Background:
- Myasthenia Gravis (MG) is a rare autoimmune disorder affecting neuromuscular junctions.
- Refractory MG cases present significant treatment challenges.
- Exploring novel therapeutic agents is crucial for managing severe MG.
Purpose of the Study:
- To evaluate the efficacy and safety of cladribine in patients with refractory myasthenia gravis.
- To assess the impact of cladribine on clinical scores and concomitant medication use.
Main Methods:
- Prospective, open-label study design.
- 13 patients with refractory MG received cladribine.
- Clinical response guided repetitive cladribine cycles and steroid dosage.
Main Results:
- 11 out of 13 patients (85%) achieved significant clinical improvement.
- Mean Myasthenia Gravis Composite (MGC) score decreased from 15.1 to 6.3 within 4 months.
- Prednisolone dosage reduced from 9.5 mg to 1.9 mg; no IVIg or PLEX needed; no adverse events reported.
Conclusions:
- Cladribine demonstrates safety and efficacy as an emergency therapy for refractory MG.
- This treatment offers a promising alternative for severe, treatment-resistant myasthenia gravis cases.
- Further research may establish cladribine's role in long-term MG management.
