  • A 14-year-old male with myopia presented with vision loss due to steroid-induced glaucoma and cataract.
  • Previous phacotrabeculectomy in the left eye and ongoing topical glaucoma medications in both eyes.

Observation:

  • The patient experienced painless, progressive vision decline in the right eye over six months.
  • Phacotrabeculectomy was performed on the right eye under general anesthesia with mannitol infusion.

Findings:

  • The surgery was uneventful, but hypotony maculopathy developed postoperatively.
  • Conservative management was initiated for the hypotony maculopathy.

Implications:

  • Phacotrabeculectomy is a viable surgical option for complex glaucoma.
