Summary

Implementing Patient Blood Management (PBM) in vascular surgery significantly reduced blood transfusions. This quality improvement program is feasible and improves transfusion practices, though hemoglobin thresholds remained unchanged.

Area of Science:

  • Transfusion Medicine
  • Vascular Surgery
  • Quality Improvement

Background:

  • Vascular surgery is a major non-cardiac consumer of blood transfusions.
  • Blood transfusions in this patient population are linked to increased risks.
  • Patient Blood Management (PBM) is a structured approach to optimize transfusion practices.

Purpose of the Study:

  • To evaluate the feasibility of implementing a PBM program in a vascular surgery setting.
  • To assess the impact of PBM on blood transfusion rates and related outcomes.
  • To determine if PBM influences hemoglobin trigger thresholds for transfusion.

Main Methods:

  • A multidisciplinary PBM program was developed and implemented by experts.
  • Educational interventions and a PBM checklist were provided to clinical staff.
  • Prospective audits were conducted before and after PBM implementation to compare outcomes.

Main Results:

  • A significant reduction in the percentage of patients receiving blood transfusions was observed (37% to 20%; p=0.01).
  • The total number of blood units transfused decreased substantially between the audit periods.
  • No significant difference was found in the hemoglobin trigger threshold used for transfusions (p=0.051).

Conclusions:

  • Patient Blood Management programs are feasible to implement within vascular surgery units.
  • PBM implementation led to improved transfusion use and potentially shorter patient stays.
  • Further research is warranted to explore the broader benefits of PBM in vascular surgery.
