    • Tacrolimus C0 dropped from 7.9 +/- 2 ng/mL to 2.6 ng/mL within one day of starting rifampin.
    • Further reduction to 1.6 ng/mL was observed with a dose increase of tacrolimus.
    • This indicates a rapid and potent drug interaction.

    Implications:

    • Early and frequent tacrolimus C0 monitoring is essential when co-prescribed with rifampin.
    • This interaction necessitates prompt dose adjustments to maintain therapeutic levels.
    • Clinical vigilance is required to manage tacrolimus-rifampin interactions in transplant patients.
