To avoid tunnel collision during combined anterior cruciate ligament (ACL) reconstruction and anterolateral tenodesis (ALT), surgeons should drill the ACL tunnel with an inclination of at least 20° anteriorly. This technique minimizes interference between the ACL and ALT femoral tunnels.
