Background:

  • Ab interno trabeculotomy (AbIT) using a Trabectome is a surgical option for open-angle glaucoma.
  • Predicting the success of AbIT is crucial for patient management and surgical planning.

Purpose of the Study:

  • To identify preoperative factors that predict the success of ab interno trabeculotomy (AbIT) performed with a Trabectome.

Main Methods:

  • A retrospective, single-center cohort study analyzed 72 eyes of 67 patients with primary open-angle or exfoliation glaucoma.
  • Patients underwent AbIT with preoperative intraocular pressure (IOP) ≥18 mmHg and were followed for at least 1 year.
  • Success was defined by a ≥20% IOP reduction or decreased medication without IOP rise or further surgery.

Main Results:

  • A history of prior selective laser trabeculoplasty (SLT) and a preoperative axial length (AL) exceeding 23.82 mm were significantly associated with decreased AbIT survival.
  • Preoperative SLT had a hazard ratio of 2.99 (P=0.013), and AL > 23.82 mm had a hazard ratio of 5.75 (P<0.001).

Conclusions:

  • Previous SLT treatment and longer axial length (>23.82 mm) are key preoperative factors negatively impacting the long-term success of AbIT.
  • These findings aid in patient selection and expectation setting for AbIT surgery in glaucoma.
