This case study presents a rare giant retroperitoneal liposarcoma with multilocular cysts. The findings highlight the importance of considering liposarcoma in the differential diagnosis of retroperitoneal cystic masses.

  • Giant retroperitoneal masses can present with non-specific symptoms like abdominal distention.

Observation:

  • A 60-year-old man presented with a 5-month history of abdominal distention.
  • Imaging revealed a giant, lobulated, cystic retroperitoneal mass with fatty components.
  • MRI showed characteristic signal intensities on T1- and T2-weighted images.

Findings:

  • Histological diagnosis confirmed a well-differentiated liposarcoma with multilocular cysts.
  • The cysts contained a rare mixture of bloody, serous, and mucinous fluids.
  • Surgical resection was performed, with tumor isolation complicated by adherence to the left external iliac artery.
