Emphysematous pyelonephritis (EPN) management has evolved. This study shows conservative treatment is effective for early-grade EPN, with no mortality observed in this case series.

Background:
- Emphysematous pyelonephritis (EPN) is a severe necrotizing kidney infection.
- Historically, nephrectomy was the primary treatment with high mortality rates (40-50%).
- This case series evaluates contemporary EPN management strategies.
Observation:
- Twenty EPN patients were analyzed retrospectively (January 2010 - February 2019).
- Patients were classified using the Huang Tseng Classification (HTC).
- Biochemical parameters and clinical outcomes were recorded.
Findings:
- No mortality was recorded in this series.
- 55% of patients (11/20) were successfully treated with intravenous antibiotics alone.
- Eight patients required interventions, including drain placement or nephrectomy, with favorable outcomes.
Implications:
- Conservative management is viable for HTC Grade-1 and Grade-2 EPN.
- Prompt escalation to intervention is crucial for higher-grade EPN with poor response.
- This approach reduces mortality and morbidity associated with EPN.
