  • Metabolic inactivity in ELT, due to inefficient vascular and biliary systems, can mimic occult metastases.
  • Gallbladder ELT is typically managed with en bloc resection via laparoscopic cholecystectomy.

Implications:

  • While laparoscopic cholecystectomy is often sufficient, a 3% risk of malignant degeneration necessitates consideration for repeat surgery to ensure negative margins and regional lymphadenectomy.
  • Awareness of ELT's vascular anatomy and malignant potential is crucial for surgical planning and patient outcomes.
