Background:

  • Liposuction is a primary treatment for solid predominant extremity lymphedema.
  • The classic liposuction technique for lymphedema does not address excess skin post-procedure.
  • Spontaneous skin contracture is the presumed resolution for post-liposuction skin excess.

Purpose of the Study:

  • To investigate the efficacy of combining liposuction with immediate skin excision for lymphedema.
  • To compare outcomes of modified liposuction with skin excision (mLIPO) versus standard liposuction (sLIPO).

Main Methods:

  • Patients with solid predominant extremity lymphedema were selected based on the "flying squirrel" sign.
  • Two groups were formed: modified liposuction with skin excision (mLIPO) and standard liposuction without skin excision (sLIPO).
  • Surgical outcomes and postoperative complications were compared between the mLIPO and sLIPO groups.

Main Results:

  • The study included 15 patients in the sLIPO group and 26 in the mLIPO group.
  • mLIPO demonstrated a statistically significant reduction in seroma/hematoma and contour irregularity.
  • mLIPO also showed a significant decrease in skin necrosis and higher patient procedural satisfaction.

Conclusions:

  • Simultaneous skin excision with liposuction for lymphedema is a safe procedure.
  • This combined technique leads to decreased postoperative complications.
  • The modified approach improves overall surgical outcomes for lymphedema patients.
