Summary

Obesity (BMI) impacts robotic partial nephrectomy (RPNx) outcomes. Post-operative complication rates decrease with higher BMI below 30, but increase significantly above this threshold, affecting underweight, normal, and morbidly obese patients.

Background:

  • Robotic assisted laparoscopic partial nephrectomy (RPNx) is standard for small renal masses.
  • Data on obesity's impact on RPNx perioperative outcomes are conflicting.

Purpose of the Study:

  • To investigate the association between Body Mass Index (BMI) and post-operative complications (POC) after RPNx.

Main Methods:

  • A cohort of 251 adult patients undergoing RPNx was analyzed.
  • Electronic medical records were reviewed for POC within 90 days.
  • A piecewise logistic regression model analyzed POC rates across BMI categories, with an inflection point at 30 kg/m².

Main Results:

  • BMI significantly correlated with POC rates.
  • POC rates decreased with increasing BMI below 30 kg/m² (OR 0.848).
  • POC rates increased with BMI above 30 kg/m² (OR 1.102).

Conclusions:

  • BMI is associated with post-operative complications in RPNx patients.
  • Underweight, normal BMI, and morbidly obese patients exhibited higher POC rates.
  • BMI should be considered in RPNx management; further research on larger cohorts is needed.
