Summary

Individuals with insomnia require longer continuous sleep to perceive sleep onset compared to controls. A new metric, the Sleep Fragment Perception Index, quantifies this sleep misperception.

Background:

Purpose of the Study:

  • To validate a model linking uninterrupted sleep bouts to sleep onset misperception using a large dataset.
  • To introduce a novel metric for quantifying sleep state misperception.

Main Methods:

  • Utilized polysomnography and sleep diaries from 139 insomnia patients and 92 controls.
  • Modeled subjective sleep onset based on the first sleep fragment exceeding a minimum duration (Ls).
  • Compared model parameters across insomnia subgroups and assessed model performance using root mean square error (RMSE).

Main Results:

  • Insomnia patients needed a median of 34 minutes of undisturbed sleep versus 22 minutes for controls to perceive sleep onset.
  • Sleep onset misperceivers required a median of 40 minutes compared to 20 minutes for non-misperceivers.
  • Extended models incorporating wake bout lengths showed minimal improvement.

Conclusions:

  • Insomnia patients, especially misperceivers, need longer continuous sleep fragments to perceive sleep onset.
  • The proposed Sleep Fragment Perception Index offers a valuable tool for characterizing sleep state misperception.
