Summary

The fast inverse direct aperture optimization (FIDAO) algorithm significantly speeds up volumetric-modulated arc therapy (VMAT) planning. FIDAO achieves comparable plan quality to conventional methods with reduced optimization times for VMAT.

Background:

  • Intensity-modulated radiation therapy (IMRT) optimization can be time-consuming.
  • Rotational volumetric-modulated arc therapy (VMAT) presents a more complex optimization challenge than fixed-gantry IMRT.
  • A novel fast direct aperture optimization (DAO) algorithm, FIDAO, was previously developed for fixed-gantry IMRT.

Purpose of the Study:

  • To extend and evaluate the fast inverse direct aperture optimization (FIDAO) algorithm for inverse planning of VMAT.
  • To assess the speed and plan quality of FIDAO in VMAT treatment planning compared to conventional methods.

Main Methods:

  • A prototype FIDAO algorithm was developed in MATLAB using the matRad toolkit.
  • VMAT plans were generated for the AAPM TG-119 phantom, and clinical liver and prostate cases.
  • Plans underwent aperture sequencing and arc sequencing, followed by DAO using either the prototype FIDAO or matRad's conventional DAO.

Main Results:

  • Both FIDAO and conventional DAO achieved similar plan quality.
  • FIDAO plans exhibited fewer hot spots in unspecified normal tissue.
  • FIDAO demonstrated significant speed enhancements: TG-119 (65s vs 602s), liver (25s vs 803s), and prostate (99s vs 754s).

Conclusions:

  • FIDAO shows promising speed improvements for direct aperture optimization in VMAT planning.
  • The algorithm maintains comparable plan quality while reducing optimization time.
  • FIDAO represents a valuable advancement for efficient VMAT treatment planning.
