    Summary

    Customized post copings (PC) for overdenture prostheses (OD) show good long-term survival, but success depends on prosthesis design and hygiene. Cylindrical retention is superior to ball attachments for better outcomes.

    Background:

    Purpose of the Study:

    • To evaluate the long-term (up to 20 years) outcomes of customized post copings (PC).
    • To identify factors influencing the risk of complications associated with PC-retained overdenture prostheses (OD).

    Main Methods:

    • A retrospective study of patients with PC-retained ODs in a university setting.
    • Data included dental/periodontal parameters, radiographs, OD design, and patient satisfaction (VAS).
    • Calculated 20-year cumulative survival and success rates for PCs.

    Main Results:

    • PC survival rate was 80.9% and success rate was 64.5% after a mean follow-up of 105.4 months.
    • Cylindrical retention showed significantly better outcomes than ball attachments (P=.012).
    • OD designs covering periodontal tissues had a higher risk of PC complications (OR 0.34; P=.010).

    Conclusions:

    • Customized PCs on natural abutments for OD retention are a viable treatment for partially dentate patients.
    • Proper prosthesis design and regular dental hygiene are essential for achieving long-term success with PCs.
