Blue light cystoscopy aids in diagnosing bladder cancer when standard methods fail. This technique identified multiple high-grade urothelial carcinoma lesions missed by white light cystoscopy in a patient with suspected malignancy.
Area of Science:
- Urology
- Oncology
- Medical Diagnostics
Background:
- Gross hematuria, urinary frequency, and dysuria are common urological symptoms.
- Atypical urine cytology warrants further investigation for bladder cancer.
- Standard white-light cystoscopy can miss significant bladder lesions.
Observation:
- A 90-year-old male presented with concerning urinary symptoms and atypical cytology.
- Two prior white-light cystoscopies did not reveal any visible bladder abnormalities.
- Intraoperative blue light cystoscopy was performed due to persistent high suspicion of malignancy.
Findings:
- Blue light cystoscopy successfully identified multiple suspicious lesions not seen with white light.
- Pathology confirmed high-grade urothelial carcinoma with lamina propria invasion.
- The patient received successful treatment with Bacille Calmette Guerin therapy.
Implications:
- Blue light cystoscopy enhances the diagnostic yield for bladder cancer in select cases.
- This technology can improve early detection of urothelial carcinoma.
- It offers a valuable adjunct to standard cystoscopy for patients with suspected bladder malignancy.
