Background:

  • Enhanced Recovery Protocols (ERP) are established for improving surgical recovery and reducing hospital stays.
  • While ERPs are increasingly used in liver surgery, their application in laparoscopic liver surgery (LLS) remains less explored.
  • Limited data exists on the efficacy of optimized perioperative care in LLS.

Purpose of the Study:

  • To prospectively evaluate the implementation and effectiveness of a multimodal Enhanced Recovery Protocol (ERP) in laparoscopic liver surgery (LLS).
  • To assess the safety, adherence, and impact of ERP on patient recovery and hospital stay in LLS.
  • To determine the feasibility of routine early discharge following LLS with ERP.

Main Methods:

  • A prospective study of the first 100 consecutive LLS patients implementing multimodal ERP principles.
  • Key ERP elements included opioid-sparing multimodal pain management, early mobilization, avoidance of drains/catheters, and prompt oral intake.
  • Pain management involved NSAIDs, low-dose opioids, corticosteroids, and oral ibuprofen/tramadol; multiprofessional adherence was assessed.

Main Results:

  • The study included 100 LLS patients (83% malignancy, 52% colorectal liver metastases, median age 65, ASA III).
  • Median postoperative hospital stay was 2 days, with 70% discharged by day 2 and 90% by day 3.
  • Complications were infrequent, and adherence to ERP elements was high.

  • Enhanced Recovery Protocols (ERP) can be safely and effectively implemented in laparoscopic liver surgery (LLS).
  • Good adherence to ERP principles was observed among the multidisciplinary team.
  • Routine discharge 1-2 days after LLS is a realistic and achievable goal with ERP.
