Equilibrium and Balance01:15

Equilibrium and Balance

6.0K
The inner ear assumes dual functionalities of auditory perception and equilibrium maintenance. The vestibule is the organ responsible for balance. This organ contains mechanoreceptors, specifically hair cells, endowed with stereocilia, which aid in deciphering information regarding the position and motion of our heads. Two intrinsic components, the utricle and saccule, help perceive head position, while the semicircular canals track head movement. Neurological messages initiated in the...

The Vestibular System01:29

The Vestibular System

43.1K
The vestibular system is a set of inner ear structures that provide a sense of balance and spatial orientation. This system is comprised of structures within the labyrinth of the inner ear, including the cochlea and two otolith organs—the utricle and saccule. The labyrinth also contains three semicircular canals—superior, posterior, and horizontal—that are oriented on different planes.

Summary

Vestibular migraine patients show normal vestibular function on the video-head impulse test during symptom-free periods. However, they exhibit higher gain in vertical canals and report increased dizziness.

Area of Science:

  • Neurology
  • Ophthalmology
  • Vestibular System Research

Background:

  • Vestibular migraine, described in 1999, lacks established pathophysiology.
  • Advancements in vestibular function assessment, such as the video-head impulse test (vHIT), aid research.
  • vHIT measures vestibular-ocular reflex gain to evaluate vestibular function.

Purpose of the Study:

  • To assess vestibular function in vestibular migraine patients using the vHIT.
  • To compare vHIT findings between vestibular migraine patients and healthy controls.

Main Methods:

  • A cross-sectional case-control study design was employed.
  • Participants included vestibular migraine patients meeting Barany Society/International Headache Society criteria.
  • The video-head impulse test was administered during the interictal period.

Main Results:

  • 31 vestibular migraine patients (90.3% female, mean age 41) were evaluated.
  • Overall vestibular function appeared normal in both groups.
  • Higher gain values were observed in vertical semicircular canals of vestibular migraine patients (p<0.05).
  • Vestibular migraine patients reported significantly more dizziness during the vHIT (p<0.001).

Conclusions:

  • Vestibular migraine patients exhibit normal vestibular function on vHIT between attacks.
  • Elevated gain in vertical canals is a notable finding in vestibular migraine.
  • Increased dizziness perception during vHIT is characteristic of vestibular migraine.
