Summary

This study introduces a new method to quantify volume conduction in Cortico-cortical evoked potentials (CCEPs) using stimulation artefact in SEEG monitoring. Findings show stimulation artefact can accurately measure volume conducted components in epilepsy patients.

Background:

  • Cortico-cortical evoked potentials (CCEPs) infer cortical connectivity using low-frequency stimulation during SEEG monitoring for epilepsy.
  • Volume conducted components of CCEPs are often overlooked and may inaccurately inflate connectivity measures.

Purpose of the Study:

  • To develop and validate a novel method for quantifying volume conducted potentials in SEEG-CCEPs.
  • To assess the role of stimulation artefact in measuring volume conduction.

Main Methods:

  • A new method quantified stimulation artefact by measuring peak-to-peak voltage in the first 10 ms post-stimulation.
  • Early CCEP responses (10-100 ms) were quantified using root mean square.
  • Artefact and responses were regressed against distance, waveform, intensity, and tissue type.

Main Results:

  • Both stimulation artefact and early CCEP responses strongly correlated with the inverse square of the distance from the stimulating electrode.
  • After distance correction, a linear relationship emerged between stimulation artefact and CCEP responses, indicating volume conduction.

Conclusions:

  • This study is the first to use stimulation artefact to quantify volume conducted potentials in SEEG.
  • The findings suggest stimulation artefact is a reliable measure for quantifying volume conducted components in CCEPs.
