Male Sexual Response: Erection & Ejaculation01:17

Male Sexual Response: Erection & Ejaculation

15.3K
Sexual stimulation can take various forms, such as physical touch and visual or auditory cues. When this happens, the parasympathetic reflex in the sacral portion of the spinal cord is activated. This reflex stimulates the release of nitric oxide (NO), which then dilates the arterioles in the penis, increasing blood flow to the erectile tissues - the corpora cavernosa and corpus spongiosum.
The blood filling the erectile tissues compresses the veins, which helps to prevent blood from leaving...

Disorders of the Male Reproductive System

3.9K
Men's health issues are increasingly recognized as significant, with several conditions posing common threats. Among these, testicular cancer is especially prevalent in younger men, particularly those aged 20 to 35 years. The disease often manifests as a painless mass in the testicles, sometimes accompanied by a sensation of heaviness or a dull ache.
Prostate disorders are another major concern. These conditions can impair urinary flow due to the prostate's location around the urethra....

Treatment for Pulmonary Arterial Hypertension: Phosphodiesterase Inhibitors

454
Phosphodiesterase 5 (PDE5) inhibitors are potent enzymes that function to hydrolyze cyclic nucleotides to their corresponding 5' monophosphates. Their unique biochemical properties have been applied in treating Pulmonary Arterial Hypertension (PAH).
Among the PDE5 inhibitors, sildenafil (Revatio) stands out as a competitive and selective inhibitor. It operates by elevating cellular levels of cGMP and augmenting signaling through the cGMP-PKG pathway, promoting vasodilation. Upon oral...

Sperm Transport01:15

Sperm Transport

2.6K
The journey of sperm from its origin to the point of ejaculation begins within the seminiferous tubules of the testis. Here, Sertoli cells produce fluid that propels non-motile sperm through a series of conduits, starting with the straight tubules leading to the rete testis. This interconnected network of tubules acts as the initial pathway for sperm, guiding them into the efferent ductules and then into the epididymis for maturation.
The maturation phase occurs in the epididymis, where sperm...

Infertility in Males01:23

Infertility in Males

485
Male infertility affects millions of couples worldwide, arising from various factors that impact different stages of the reproductive process. An endocrine imbalance resulting from conditions like hypogonadism, Klinefelter syndrome, or pituitary disorders can disrupt hormone levels and reduce sperm production. Testicular defects, such as tumors, cryptorchidism, atrophic testes, abnormal sperm morphology, and low sperm count or motility, may arise due to genetic factors, structural...

Factors Affecting Drug Distribution: Physiological Barriers

620
Drug distribution in the body is intricately regulated by various physiological barriers that control the passage of substances. These include the capillary endothelial barrier, the blood-brain, blood-cerebrospinal fluid, blood-placental, and blood-testis barriers.
The capillary endothelial barrier allows only smaller molecules below 600 Da (Daltons) to pass through. It also restricts drugs like heparin that are bound to blood components, limiting their movement within the bloodstream.
The...

Summary

Premature ejaculation (PE) is the most common male sexual dysfunction but remains underdiagnosed. Current treatments effectively improve sexual satisfaction by increasing intravaginal ejaculatory latency time, though not curatively.

Area of Science:

  • Urology
  • Sexual Medicine
  • Reproductive Health

Background:

  • Premature ejaculation (PE) is a prevalent male sexual dysfunction.
  • Accurate diagnosis is crucial for effective management.
  • Understanding PE's physiology is key to developing treatments.

Purpose of the Study:

  • To conduct a comprehensive literature review on premature ejaculation.
  • To cover the spectrum from physiological understanding to current treatment modalities.
  • To synthesize current knowledge for clinical application.

Main Methods:

  • Systematic literature search using PubMed and Cochrane databases.
  • Review focused on physiological aspects, diagnostic criteria, and therapeutic options.
  • Analysis of studies evaluating treatment efficacy and patient outcomes.

Main Results:

  • Clinical history is the primary diagnostic tool for differentiating primary and acquired PE.
  • Current treatments aim to improve sexual satisfaction and intravaginal ejaculatory latency time (IELT).
  • Treatment strategies are not curative but offer significant symptomatic relief.

Conclusions:

  • Premature ejaculation is frequently underdiagnosed despite its high prevalence.
  • A combined approach using behavioral techniques and pharmacotherapy offers the most effective treatment strategy.
  • Improving IELT and sexual satisfaction are key goals in managing PE.
