Summary

Laser interstitial therapy shows promise for treating brainstem cavernous malformations (CMs), reducing hemorrhage risk and improving patient symptoms. Early results suggest technical safety, but further research is needed.

Background:

  • Brainstem cavernous malformations (CMs) present significant risks, including high hemorrhage rates and severe neurological deficits.
  • Recurrent hemorrhages in pontine CMs necessitate effective and safe treatment options.

Observation:

  • Two patients with pontine CMs experienced recurrent hemorrhages.
  • Magnetic resonance thermography-guided laser interstitial therapy was employed for treatment.

Findings:

  • Both patients demonstrated substantial symptomatic improvement post-treatment.
  • Patients remained hemorrhage-free at 12- and 6-month follow-ups.
  • Radiographic imaging confirmed lesion involution in both cases.

Implications:

  • Laser interstitial therapy may offer a technically safe approach for managing brainstem CMs.
  • Larger studies and extended follow-up are crucial to establish definitive efficacy.
  • This minimally invasive technique could represent a valuable therapeutic option for select patients.
