Issues And Trends In Healthcare Delivery System

6.1K
The issues and trends in healthcare delivery are constantly changing. The COVID-19 pandemic is one recent issue that wreaked havoc on healthcare systems, causing a shortage of healthcare workers, high demand for medicines and supplies, and increased medical expenditure due to a lack of insurance. Other issues include rising healthcare costs and care fragmentation.

Introduction To Health Care Delivery System

3.6K
The healthcare system is constantly changing and complex. Various services are available from different healthcare providers, but gaining access to these services has become challenging for people with limited healthcare insurance. Uninsured people present a challenge to healthcare because they frequently postpone or forego treatment.

Data Collection I01:30

Data Collection I

7.7K
Data collection gathers information needed to make accurate judgments about a patient's present condition. During a health history interview, subjective data is collected from the patient, their caregivers, or family members, and objective data is collected through observations and physical assessment. Patients are the primary source of subjective data. Thus information gathered from patients through interviews, observations, and physical examination is primary data. Secondary sources of...

Summary

Successful patient-reported outcomes (PROs) collection is linked to physician training, administrative oversight, and clinical champions. Previous paper-based PRO collection also improved rates in a large healthcare system.

Area of Science:

  • Health Services Research
  • Clinical Informatics
  • Patient Outcomes

Background:

  • Patient-reported outcomes (PROs) collection is crucial but faces limited dissemination and analysis of success factors.
  • Centralized PRO infrastructure can potentially streamline collection processes within large healthcare systems.

Purpose of the Study:

  • To identify key factors associated with improved PRO collection rates.
  • To analyze the impact of a centralized PRO infrastructure in a large healthcare system.

Main Methods:

  • A cohort study analyzed data from 205 medical and surgical clinics within the Partners Healthcare system (2014-2018).
  • Linear and logistic regression models were used to assess the relationship between clinic characteristics and PRO collection rates (primary outcome: mean collection rate over 6 months).
  • Successful clinics were defined as those achieving >50% PRO collection rate.

Main Results:

  • Over 4 million PRO measures were collected from over 745,000 encounters.
  • Clinics with >50% physician training on PROs showed a 19.6% increase in collection rates.
  • Routine administrative oversight, prior paper-based PRO collection, clinical champions, and payer incentives were also associated with increased collection rates.

Conclusions:

  • Physician training, administrative oversight, and clinical champions are promising interventions for enhancing PRO collection.
  • Clinics with prior experience in PRO collection may achieve higher success rates.
  • While payer incentives improved collection, they did not correlate with overall program success.
