Background:

  • Post-marketing surveillance indicated potential depression risk with 5α-reductase inhibitors (finasteride, dutasteride).

Purpose of the Study:

  • To investigate the association between 5α-reductase inhibitor exposure and the risk of depression.
  • To evaluate depression risk using sequence symmetry analysis in a large patient cohort.

Main Methods:

  • Utilized National Veterans Health Administration administrative data from fiscal year 2014.
  • Identified 53,848 male patients initiating 5α-reductase inhibitor therapy.
  • Assessed incident depression via antidepressant prescription and diagnosis, comparing pre- and post-initiation periods using symmetry ratios. Included a counterfactual analysis with α1-adrenergic receptor antagonists.

Main Results:

  • No increased incidence of antidepressant prescribing or depression diagnosis was observed following 5α-reductase inhibitor initiation (SR 0.84 and 0.83, respectively).
  • Stratification by various factors (age, drug, antidepressant class, prior exposure) did not reveal a positive association.
  • An α1-adrenergic receptor antagonist analysis yielded similar null results (SR 0.87).

Conclusions:

  • Initiation of 5α-reductase inhibitors is not associated with an elevated risk of depression.
  • Findings suggest that depression in patients using these drugs may be more related to underlying benign prostatic hyperplasia and its symptoms rather than the medication itself.
