Rapidly dividing tumors, embryos, and wounded tissues require more oxygen than usual, lowering the oxygen concentration in the blood. At low oxygen or hypoxic conditions, an oxygen-sensitive transcription factor called the hypoxia-inducible factor 1 or HIF1 is activated. Summary

This case report highlights a rare instance of gigantic idiopathic angioid streaks, potentially exacerbated by past steroid abuse. Advanced imaging confirmed the diagnosis and revealed extensive retinal involvement.

Background:

  • Idiopathic angioid streaks are a rare condition affecting Bruch's membrane.
  • Macular choroidal neovascularization (CNVM) and peripheral retinal hemorrhages are potential complications.

Observation:

  • A young male presented with CNVM and peripheral retinal hemorrhages.
  • Gigantic angioid streaks extending to the equator were observed in both eyes.
  • Breaks in the retinal pigment epithelium-Bruch's membrane complex were noted.

Findings:

  • Swept source optical coherence tomography (SSOCT) and ultrawide field imaging were crucial for diagnosis.
  • A history of steroid abuse was the only significant systemic finding.
  • The angioid streaks were idiopathic in origin.

Implications:

  • Advanced imaging is vital for diagnosing and characterizing extensive angioid streaks.
  • The potential role of steroid abuse in angioid streak presentation warrants further investigation.
  • This case expands understanding of the clinical spectrum of idiopathic angioid streaks.
