Background:

  • Erectile dysfunction (ED) is a common condition with various underlying causes.
  • Assessing the microvasculature may offer insights into ED pathophysiology.

Purpose of the Study:

  • To investigate retinal microvascular changes using optical coherence tomography angiography (OCT-A) in patients with ED.
  • To determine if OCT-A parameters can assist in the differential diagnosis of ED subtypes.

Main Methods:

  • Prospective, observational, cross-sectional study.
  • OCT-A assessed vessel densities (VD) in superficial capillary plexus (SCP), deep capillary plexus (DCP), and retinal peripapillary capillary plexus (RPCP).
  • Patients categorized into organic ED, psychogenic ED, and healthy control groups based on clinical evaluation and penile Doppler ultrasonography (PDU).

Main Results:

  • Significantly lower VDs in DCP, SCP, and temporal RPCP were observed in the organic ED group compared to psychogenic ED and healthy controls.
  • SCP and DCP VDs correlated with penile cavernous artery peak systolic velocity.
  • DCP VDs correlated with penile resistive indexes.

Conclusions:

  • OCT-A, particularly assessing DCP and SCP, shows potential in evaluating ED.
  • OCT-A data may provide valuable information regarding the origin of ED, complementing other diagnostic methods.
