Summary

Anaplastic large cell lymphoma (ALCL) presenting as a dural-based mass with bone invasion is a rare form of primary central nervous system lymphoma (PCNSL). This case highlights a unique presentation and successful treatment in a pediatric patient.

Background:

  • Primary central nervous system lymphoma (PCNSL) typically presents as an intraparenchymal mass.
  • Meningeal and bone involvement are uncommon in PCNSL.

Observation:

  • A 12-year-old boy presented with headache and seizures.
  • MRI revealed a dural-based mass with osteolytic bone changes.
  • Surgical resection showed a hard, dural-based tumor with necrosis and hemorrhage, invading the skull.

Findings:

  • Pathological examination identified large atypical cells positive for CD30, epithelial cell antigen, and anaplastic lymphoma kinase.
  • The final diagnosis was anaplastic lymphoma kinase-positive anaplastic large cell lymphoma (ALCL).
  • No systemic cancer was detected.

Implications:

  • Central nervous system ALCL frequently involves the dura and meninges but rarely causes osteolytic bone lesions.
  • This case represents a rare instance of dural-based ALCL with bone invasion.
  • The patient achieved remission after chemotherapy, suggesting effective treatment protocols for this rare presentation.
