Summary

A rare kidney triplication with ureterocele in a child was successfully treated with minimally invasive surgery. This approach preserved upper moiety kidney function, resolving urinary incontinence and avoiding nephrectomy.

Background:

  • Kidney triplication is a rare congenital anomaly.
  • The co-occurrence of triplex kidney and ureterocele is exceptionally uncommon.
  • Standard treatment often involves heminephrureterectomy of the affected upper moiety.

Observation:

  • A 5-year-old girl presented with continuous urinary incontinence.
  • Diagnostic imaging revealed a 3-segmented left kidney with pelvi-ureteric dilation and Grade IV vesicoureteral reflux in the upper moiety.
  • The upper moiety had a cervical ectopic ureteral orifice, while lower segments shared a common orifice.

Findings:

  • Endoscopic laser dissection of the ureterocele successfully restored drainage to the upper moiety.
  • Post-procedure follow-up at 18 months showed resolution of incontinence and infection symptoms.
  • Pelvi-ureteric dilation and ureterocele cavity significantly reduced, with vesicoureteral reflux decreasing to Grade I.

Implications:

  • Minimally invasive endoscopic treatment can effectively manage upper moiety obstruction in triplex kidneys.
  • This approach successfully preserved renal function, obviating the need for heminephrectomy.
  • Successful management led to the regression of vesicoureteral reflux and urinary incontinence.
