Background:

  • Insulinomas are neuroendocrine tumors of pancreatic beta cells, requiring accurate localization for curative surgical resection.
  • Distinguishing insulinomas from congenital hyperinsulinism is critical for appropriate management.

Purpose of the Study:

  • To evaluate the diagnostic utility of glucagon-like peptide 1 receptor (GLP-1R) PET/CT using Ga-DOTA-exendin 4 for detecting insulinomas.
  • To compare the diagnostic performance of PET scans acquired at 1 hour (PET1) and 2 hours (PET2) post-injection.

Main Methods:

  • Ten patients with suspected insulinoma underwent Ga-DOTA-exendin 4 PET/CT at 1 and 2 hours post-injection.
  • Two independent readers visually assessed tracer uptake, and quantitative metrics (SUVmax, TBR) were analyzed.
  • Imaging findings were correlated with histopathology in surgically resected cases.

Main Results:

  • Focal Ga-DOTA-exendin 4 uptake was observed in 8 out of 10 patients, confirmed as insulinoma in 7 post-surgery.
  • Two patients without focal uptake were diagnosed with diffuse congenital hyperinsulinism.
  • While SUVmax increased significantly from PET1 to PET2, tumor-to-background ratio did not show significant improvement.

Conclusions:

  • Focal Ga-DOTA-exendin 4 uptake on PET/CT accurately identifies insulinomas.
  • A single 1-hour PET/CT scan (PET1) provides sufficient diagnostic information, making it a convenient approach for patient care.
