Summary
Tafasitamab plus lenalidomide showed a 60% objective response rate in patients with relapsed or refractory diffuse large B-cell lymphoma ineligible for stem-cell transplantation. This combination therapy offers a promising new option for this challenging patient group.
Area of Science:
- Hematology
- Oncology
- Immunotherapy
Background:
- Relapsed or refractory diffuse large B-cell lymphoma (DLBCL) in patients ineligible for autologous stem-cell transplantation (ASCT) is associated with poor outcomes.
- Tafasitamab (MOR208), an anti-CD19 monoclonal antibody, has demonstrated activity in B-cell malignancies.
- Preclinical studies suggested a synergistic effect between tafasitamab and lenalidomide.
Purpose of the Study:
- To evaluate the antitumour activity and safety of combining tafasitamab with lenalidomide.
- To assess this combination as a potential therapeutic option for patients with relapsed/refractory DLBCL ineligible for ASCT.
Main Methods:
- A multicentre, open-label, single-arm, phase 2 study (L-MIND) was conducted.
- Eligible patients received intravenous tafasitamab and oral lenalidomide for up to 12 cycles.
- The primary endpoint was the objective response rate (ORR) assessed centrally.
Main Results:
- An objective response was observed in 60% (48/80) of patients, with 43% achieving a complete response.
- The most frequent grade 3 or worse adverse events included neutropenia (48%) and febrile neutropenia (12%).
- Serious adverse events occurred in 51% of patients, with pneumonia and febrile neutropenia being the most common.
Conclusions:
- Tafasitamab plus lenalidomide demonstrated significant antitumour activity in patients with relapsed/refractory DLBCL ineligible for ASCT.
- The combination was generally well-tolerated and may represent a new therapeutic option for this underserved population.
