Summary

Recurrent patellar dislocation risk is higher in younger patients and those with anatomical factors like patella alta. Identifying these risk factors helps determine if conservative or surgical treatment is best for primary patellar dislocation.

Purpose of the Study:

  • To systematically review and identify risk stratification models for recurrent dislocation after primary patellar dislocation.
  • To determine key risk factors influencing the recurrence of patellar dislocations.

  • A systematic literature search was conducted using OvidSP and MEDLINE.
  • Ten studies were included to identify risk factors and stratification models for recurrent patellar dislocation.

  • Eight risk factors for recurrence were identified, with younger age (under 16) and anatomical factors being significant.
  • Patella alta (OR 4.259) and trochlear dysplasia were major anatomical risk factors.

  • Patients with multiple combined risk factors exhibited a very high risk of recurrence.

Conclusions:

  • Recurrent dislocation risk is elevated by specific risk factors, especially when multiple are present.
  • Published stratification models aid in assessing individual recurrence risk.
  • Conservative management is suitable for low-risk patients; surgical intervention should be considered for high-risk individuals.
