Insights
Children with migraine show abnormal ambulatory blood pressure monitoring (ABPM) patterns, indicating potential hypertension risks. ABPM is recommended for pediatric migraine patients, even with normal office blood pressure readings.
Area of Science:
- Pediatric Cardiology
- Neurology
- Hypertension Research
Background:
- Limited research exists on hypertension in children with migraine.
- Adult studies suggest a link between migraine and increased hypertension frequency.
Purpose of the Study:
- To investigate hypertension prevalence in children experiencing migraines.
- To utilize ambulatory blood pressure monitoring (ABPM) for comprehensive assessment.
Main Methods:
- Compared 37 children with migraine to 30 healthy controls.
- Recorded demographic, clinical, and physical examination data.
- Performed office blood pressure measurements and ABPM for all participants.
Main Results:
- Abnormal ABPM patterns were significantly more frequent in the migraine group (45.9%) versus controls (16.7%).
- Children with migraine exhibited higher nighttime mean arterial and diastolic blood pressure.
- A higher prevalence of the non-dipping blood pressure pattern was observed in the migraine group.
Conclusions:
- Nearly half of pediatric migraine patients may have ambulatory blood pressure abnormalities.
- ABPM is recommended for children with migraine, irrespective of normal office blood pressure readings.
Background:
Although there are data showing that the frequency of hypertension increases in adults with migraine, there has been no study on this subject in children. In this study, we aimed to evaluate the presence of hypertension in children with migraine by performing ambulatory blood pressure monitoring (ABPM).
Methods:
Thirty-seven children diagnosed with migraine and 30 healthy controls were evaluated between January 2015 and March 2016. Demographic data, clinical and laboratory features, and physical examination findings were recorded for both groups. Office blood pressure was measured for all children, and each also underwent ABPM. The two groups were compared in terms of ambulatory blood pressure parameters.
Results:
The mean age was 13.3 and 13.1 years and the proportion of females was 73% and 60% in the migraine and control groups, respectively. Although the frequency of hypertension was not higher, abnormal ABPM patterns were found to be significantly more frequent in the migraine group (migraine, 45.9%; control, 16.7%; p, 0.018). Nighttime mean arterial blood pressure, nighttime diastolic blood pressure, and non-dipping pattern were higher in children with migraine than those in the control group (p < 0.05).
Conclusions:
These results suggest that ambulatory blood pressure abnormalities may be present in almost half of patients with migraine. Therefore, we suggest that ABPM should be performed even if the office blood pressure measurements of children diagnosed with migraine are normal.

Alterations in Blood Pressure
Hypertension (High blood pressure)
Hypertension occurs when blood pressure readings consistently exceed the normal range. It is diagnosed when systolic blood pressure (the top number, indicating pressure while the heart...
Special considerations while measuring blood pressure
Monitoring Both Arms:
Monitoring BP in both arms during the initial assessment is advisable, as the systolic value may differ by five to ten mm Hg between arms. For subsequent BP assessments, use the arm with the higher reading.

Blood Pressure
Blood Pressure
The average BP in an adult is typically around 120/80 mmHg (millimeters of mercury). In this measurement, the numerator (120) indicates the systolic pressure, which is the pressure in the arteries during the contraction of the heart's ventricles as blood is expelled. The denominator (80) represents the...
