Ankle Joint01:10

Ankle Joint

2.5K
The ankle is formed by the talocrural joint (crural = leg). It consists of the articulations between the talus bone of the foot and the distal ends of the tibia and fibula of the leg. The superior aspect of the talus bone is square-shaped and has three areas of articulation. The top of the talus articulates with the inferior tibia. This is the portion of the ankle joint that carries the body weight between the leg and foot. The sides of the talus are firmly held in position by the articulations...

Area of Science:

  • Orthopedic surgery
  • Traumatology
  • Reconstructive surgery

Background:

  • Subtalar dislocations are infrequent injuries, representing 1-2% of all major joint dislocations.
  • Chronic subtalar dislocations are rarely documented for reconstruction.
  • This report introduces the Ilizarov frame for chronic subtalar dislocation reconstruction.

Observation:

  • A 67-year-old female patient presented with a decade-long history of worsening ankle pain and varus deformity.
  • Imaging revealed medial subtalar and talonavicular joint dislocations with talar fractures and osteoarthritis.
  • Severe contractures significantly impaired ambulation.

Findings:

  • A two-stage reconstruction utilizing an Ilizarov frame followed by arthrodesis was performed.
  • This treatment successfully corrected both bony deformities and soft tissue contractures.
  • The patient regained independent ambulation post-treatment.

Implications:

  • Soft tissue management is critical in subtalar dislocation reconstruction, alongside bony correction.
  • The Ilizarov frame is effective for addressing complex bony and soft tissue deformities.
  • Successful outcomes require surgical expertise and patient motivation for complex reconstructions.
