Autism spectrum disorder (ASD) is a neurodevelopmental condition marked by persistent deficits in social communication and interaction alongside restrictive and repetitive behaviors or interests. ASD is sometimes accompanied by intellectual impairment.
These core symptoms manifest differently among individuals, ranging from mild to severe. The disorder's complexity extends beyond its clinical presentation, encompassing a diverse range of biological, cognitive, and sociocultural influences.

Background:

  • Congenital heart disease (CHD) affects 1.7% of US children, with a 2-5 fold increased risk for autism.
  • The Cardiac Neurodevelopmental Outcome Collaborative recommends universal autism screening for infants and adolescents with CHD.
  • This study evaluated autism concerns and clinical practices within a Cardiac Neurodevelopmental Program.

Purpose of the Study:

  • To determine the frequency of autism concerns in children referred to a Cardiac Neurodevelopmental Program.
  • To examine current clinical practices for autism screening in this population.
  • To identify areas for improving the quality of care for children with CHD and autism.

Main Methods:

  • A retrospective chart review of 134 children (mean age 9.0 years) with high-risk CHD evaluated between 2018-2019.
  • Utilized parent-report data from the Behavior Assessment System for Children-3 and Adaptive Behavior Assessment System-3.
  • Descriptive and correlational analyses were performed on collected data.

Main Results:

  • 11.2% of referred children had autism-related concerns; at least 5.9% were diagnosed with autism.
  • Atypicality, deficient adaptability, and impaired social/communication skills were noted.
  • Data on early intervention, school supports, and comorbidities were analyzed.

Conclusions:

  • Autism screening may not be consistently implemented in CHD clinical care prior to established recommendations.
  • The study sample revealed a high prevalence of autism in referred children.
  • Systematic autism screening is essential, as parent-report measures alone are insufficient for diagnosis.
