Mathematical modeling suggests that longer than six weeks of sofosbuvir (SOF) and ribavirin (R) therapy is needed for a sustained virologic response in recently infected hepatitis C virus (HCV) patients. Further research is required to validate response-guided therapy for recent HCV infection.
Area of Science:
- Hepatology
- Virology
- Mathematical Biology
Background:
- Mathematical modeling of viral kinetics aids in predicting treatment duration for chronic Hepatitis C Virus (HCV) patients.
- This approach has not been extensively evaluated in individuals with recent HCV infection.
Purpose of the Study:
- To retrospectively assess if mathematical modeling can predict treatment outcomes.
- To evaluate the efficacy of six-week sofosbuvir (SOF) and weight-based ribavirin (R) therapy in recently infected individuals.
Main Methods:
- Mathematical modeling was employed to estimate viral host parameters.
- The study analyzed 12 adults with recent HCV infection (<12 months).
- Participants received six weeks of SOF + R treatment.
Main Results:
- Modeling demonstrated a 100% negative predictive value for SOF + R treatment response in nine participants.
- A median of 13 weeks of therapy was predicted for cure.
- Cure was predicted for two participants who later relapsed.
Conclusions:
- Modeling indicates that over six weeks of SOF + R therapy is necessary for cure in recent HCV infection.
- Prospective, real-time modeling is needed to validate response-guided therapy for recent HCV infection management.
