Reactive arthritis can occur after Clostridium difficile infection in adults. This study found a benign course for this joint complication, even without typical reactive symptoms.

Area of Science:

  • Rheumatology
  • Infectious Diseases
  • Microbiology

Background:

  • Clostridium difficile infection (CDI) is a significant cause of infectious diarrhea.
  • Reactive arthritis is a form of inflammatory arthritis that can be triggered by infections elsewhere in the body.

Observation:

  • Two adult patients developed reactive arthritis following enteric Clostridium difficile infection.
  • Neither patient exhibited other common reactive arthritis symptoms like conjunctivitis, mucous membrane lesions, or urethritis.
  • One patient was positive for the HLA-B27 antigen.

Findings:

  • Reactive arthritis following CDI presented with peripheral joint involvement in the observed cases.
  • The arthritis followed a benign, short-term course in both patients.

Related Experiment Videos

  • Review of three additional cases of C. difficile-associated arthritis supports these observations.
  • Implications:

    • Clostridium difficile infection should be considered as a potential trigger for reactive arthritis, particularly in adults.
    • The absence of classic reactive symptoms does not rule out this diagnosis.
    • Further research is warranted to understand the pathogenesis and long-term outcomes of C. difficile-associated reactive arthritis.
