Clostridium difficile was not found to be more prevalent in children with autism spectrum disorder (ASD) compared to controls. Despite most strains being toxigenic, no significant association with gastrointestinal symptoms or autism severity was observed.

- Quantitative real-time PCR was used to detect and quantify C. difficile and its toxins A and B.
Main Results:
- C. difficile was detected in 25.9% of ASD cases, 40% of siblings, and 15.6% of controls.
- A high percentage of detected C. difficile strains were toxigenic across all groups (73.3% in ASD, 77.8% in siblings, 71.4% in controls).
- No statistically significant differences were found between the groups regarding C. difficile prevalence, quantity, or toxin production.
- No significant association was found between C. difficile and GI Severity Index in ASD cases.
Conclusions:
- Clostridium difficile is not specifically more prevalent in the gut of children with ASD compared to neurotypical siblings and unrelated controls.
- Although most identified C. difficile strains were toxigenic, their presence did not correlate with GI symptoms in controls or significantly with GI severity in ASD cases.
- Gastrointestinal dysfunction and sensory impairment remain common comorbidities in ASD, but C. difficile does not appear to be a primary driver in this cohort.
