Summary

Performing redo thyroid surgery without drains is safe and effective. This approach does not increase surgical complications and reduces hospital stays, offering a more efficient patient recovery.

Background:

  • Redo thyroid surgery carries a higher risk of complications, such as hematoma, compared to initial procedures.
  • The use of surgical drains in thyroidectomies is a common practice, but their necessity in redo procedures is debated.

Purpose of the Study:

  • To evaluate the safety and efficiency of performing redo thyroid surgery without surgical drains.
  • To compare complication rates and patient outcomes in redo thyroid surgery with and without drains.

Main Methods:

  • A retrospective single-institutional study compared three groups: primary bilateral thyroidectomy, completion thyroidectomy, and thyroidectomy for recurrent disease.
  • All patients underwent surgery without surgical drains, and outcomes were analyzed.

Main Results:

  • Hematoma incidence was low across all groups (4-5%).
  • Rates of transient hypocalcemia and recurrent laryngeal nerve (RLN) paralysis were comparable to historical data.
  • Permanent unilateral RLN paralysis occurred in 1% of patients in redo groups.
  • The majority of patients (92%) were discharged within 1 day.

Conclusions:

  • Routine drain use can be safely omitted in redo thyroid surgery.
  • Avoiding drains in redo thyroid surgery does not increase surgical morbidity.
  • This practice leads to a reduced length of hospital stay.
