Background:

Morbid obesity recurrence after laparoscopic sleeve gastrectomy is a significant clinical challenge.
Revision sleeve gastrectomy (ReSG) presents an alternative restrictive bariatric procedure.
The MIDCAL™ ring offers a novel approach to banded ReSG.

Observation:

This study details the surgical technique for placing a MIDCAL™ ring during a revised sleeve gastrectomy.
The procedure involves dissection of the prior staple line, calibration, restapling, and secure fixation of the ring.
The video demonstrates a posterior approach with a three-port technique.

Findings:

The operative outcome was favorable, with no immediate complications reported.
The patient achieved 9% total body weight loss at 1 month and 27% at 2 years post-procedure.
Banded ReSG demonstrated acceptable short-term efficacy.

Implications:

Banded ReSG is a safe and viable option for patients experiencing weight regain after sleeve gastrectomy.
Further comparative studies are warranted to establish long-term outcomes and efficacy.
This technique provides an alternative restrictive solution in bariatric surgery management.
