Carbonic anhydrase 9 (CA9) is highly expressed in clear cell renal cell carcinoma (CCRCC) and clear cell tubulopapillary RCCs, aiding diagnosis. However, CA9 expression is not entirely specific and can appear focally in other renal tumors.

- Focal or rare CA9 expression was noted in a subset of other RENs, including Xp11.2 RCC and fumarate hydratase-deficient RCC; oncocytomas and eosinophilic solid and cystic RCCs were negative.

- Diffuse CA9 expression demonstrates high sensitivity for CCRCC and CCTPRCC.
- CA9 is not entirely specific, as focal expression can occur in other renal tumor types.
- CA9 findings should be interpreted alongside morphological, immunophenotypic, and clinical data for accurate diagnosis.
